Je comprends que par la nature de l’activité, il y a possibilité d'accident et/ou blessure et que j’assume le risque et la responsabilité en participant à ce sport. Je n'apporterai pas d’action légale et je tiens non coupable le district scolaire et/ou leurs représentants, la facilité, c’est-à-dire l’école et leurs représentants et aussi les organisateurs de ce sport pour n’importe quelle accident ou blessure. Je comprends aussi que n'importe quel coût est ma responsabilité personnelle et que le district scolaire et/ou leurs représentants ainsi que les organisateurs n’ont pas d’assurance médicale pour des blessures et/ou accident subissent par un participant.

 

I understand, by the nature of the activity, that there is a possibility of an accident and/or injury, and I assume the risk and responsibility while I participate in this sport. I hold harmless the school district and/or its representatives, as well as the host facility/school and its representatives, as well as the sport organizers for any accident and/or injury that I may sustain during participation in this sport. I also forfeit legal action or compensation claims against the school district and/or its representatives, for injuries and/or accidents I may sustain. I also understand that all incurred costs are my personal responsibility, and that the school district and the host facility/school and organizers do not have medical insurance coverage for injuries and/or accidents to a participant.
